When starting this form, you may get a message about restrictions for using the active modules.
Click on the message. You will get a menu with three choices. Select "Enable blocked content" and on the message coming up answer "Yes". If not done, you may have trouble using this form. Entering all the data on the form may require a lot of writing, but help is avaliable. Click on the menu "Tools" and continue with "Internet options..." Select "Content" and go to "Auto complete". Mark the box "Forms". After this, you will not have to repeat the same info if you have written the same content in that box before. Double-click in the box, and you may select from a list.
